Physical Activity Readiness Questionnaire

The PAR-Q is a standard health-screening tool used before starting a new physical activity programme. It helps identify whether you should consult a doctor before exercising. Please read each question carefully and consider your honest answer.
Health Screening Questions
- Has your doctor ever said that you have a heart condition and that you should only do physical activity recommended by a doctor?
- Do you feel pain in your chest when you do physical activity?
- In the past month, have you had chest pain when you were not doing physical activity?
- Do you lose your balance because of dizziness or do you ever lose consciousness?
- Do you have a bone or joint problem (for example, back, knee, or hip) that could be made worse by a change in your physical activity?
- Is your doctor currently prescribing drugs (for example, water pills) for your blood pressure or heart condition?
- Do you know of any other reason why you should not do physical activity?
What Your Answers Mean
If you answered YES to any question above:
Talk to your doctor by phone or in person before starting or increasing your physical activity. Tell your doctor about the PAR-Q and which question(s) you answered YES to. You may be able to do any activity you want — as long as you start slowly and build up gradually. Or, you may need to restrict your activities to those that are safe for you.
If you answered NO to all questions:
You can reasonably be sure that you can start becoming more physically active and take part in a fitness appraisal. Begin slowly and build up gradually — this is the safest and easiest way to go.
Important Notes
- This questionnaire is valid for a maximum of 12 months from the date of completion, provided your condition has not changed.
- If you are currently pregnant or have recently given birth, please consult your doctor before participating in physical activity.
- If your health changes so that you would answer YES to any of the above questions, inform your fitness or health professional and update your answer.
- The PAR-Q is a confidential document. Club Compete will not share your answers with any third party without your consent, except where required by law.

You understand that Club Compete’s coaching programmes involve moderate-to-vigorous physical activity, and you take full responsibility for ensuring you are medically fit to participate.
Club Compete recommends that all participants complete this questionnaire honestly and seek medical advice if they have any concerns before commencing activity.
